Özet
Parkinson hastalığı (PH), ileri yaşlarda morbiditeye yol açan, dopamin eksikliği kaynaklı nörodejeneratif bir hastalıktır. Temel motor belirtileri istirahat tremoru, rijidite, bradikinezi ve postural instabilitedir. Hastalık ilerledikçe yürüme bozuklukları, festinasyon ve donma olayları yaşam kalitesini düşürür. Motor semptomların yanı sıra bilişsel bozukluklar, otonomik disfonksiyon, uyku problemleri, anksiyete, depresyon ve ağrı gibi motor olmayan semptomlar da yaygın olarak görülür. Tedavi yaklaşımında farmakolojik müdahaleler substantia nigra hücrelerindeki progresif ölümü durduramadığı için motor dalgalanmalar yaşanabilir. Bu aşamada fizik tedavi ve rehabilitasyon programları kritik bir rol oynar. Fizik tedavi hareket bozukluklarını tek başına tedavi edemese de, görsel ve işitsel ipuçları, denge ve yürüme egzersizleri gibi stratejilerle hastalara daha kolay hareket etme yollarını öğretir. Ev ve iş ortamının düşme riskine karşı düzenlenmesi ve hastanın düzenli fiziksel aktiviteye teşvik edilmesi esastır. Sonuç olarak, ilaç tedavisiyle entegre edilen rehabilitasyon programları, Parkinson hastalarında engelliliği azaltarak yaşam kalitesini ve toplumsal rollere katılımı optimize etmeyi amaçlar.
Parkinson's disease (PD) is a neurodegenerative disorder caused by dopamine deficiency, resulting in significant morbidity in older adults. Its primary motor symptoms include resting tremor, rigidity, bradykinesia, and postural instability. As the disease progresses, gait disturbances, festination, and freezing episodes severely impair the quality of life. In addition to motor deficits, patients frequently experience non-motor symptoms such as cognitive decline, autonomic dysfunction, sleep disturbances, anxiety, depression, and chronic pain. Because pharmacological treatments cannot halt the progressive degeneration of dopaminergic neurons in the substantia nigra, motor fluctuations occur over time. Consequently, physical therapy and rehabilitation programs become essential. While physiotherapy cannot cure movement disorders independently, it provides patients with effective compensatory strategies, utilizing visual and auditory cues alongside balance and gait training. Modifying home and work environments to mitigate fall risks and promoting consistent physical activity are crucial components of management. Ultimately, rehabilitation programs integrated with medical treatment aim to minimize disability, optimize functional independence, and enhance the overall quality of life for individuals with Parkinson's disease.
