Koroner Anomalisi Olan Bir Akut Koroner Sendrom Vakasında Sağ Judkins Katater Yanılgısı: Sağ Koroner Arterden (RCA) Çıkan Culprit Sol Sirkumfleks Arter (LCX)

Konjenital koroner arter anomalisi (KKAA) olan 68 yaşındaki bir kadın hasta, göğüs ağrısı ve yüksek troponin değerleriyle akut koroner sendrom kliniğiyle acil servise başvurmuştur. Koroner anjiyografi (KAG) sırasında sol sirkumfleks arterin (LCX) normal yerinde izlenememesi üzerine yapılan detaylı incelemelerde, LCX'in sağ koroner arterden (RCA) köken aldığı saptanmıştır. İlk etapta tercih edilen Sağ Judkins (JR) kateterinin uzun gaga yapısı nedeniyle LCX ostiumunu kapatması, suçlu lezyonun başlangıçta görüntülenmesini engellemiştir. Daha küçük gagalı Amplatz Left 1 (AL 1) kateter kullanıldığında ise LCX'in proksimal segmentte %100 tıkalı olduğu tespit edilmiş, ardından balon ve stent uygulamalarıyla girişimsel süreç başarıyla tamamlanmıştır. İşlem sonrası yapılan koroner BT anjiyografide LCX'in retroaortik seyir gösterdiği doğrulanmıştır. Genç erişkinlerdeki travmatik olmayan kardiyak ölümlerin önemli bir nedeni olan KKAA'lar, KAG sırasında kateter yanılgılarına yol açarak tanısal zorluklar çıkarabilmektedir. Vakada görüldüğü üzere, boş kalan koroner sahaların zamanında fark edilmesi, anatomik varyasyonların doğru analizi ve uygun alternatif kateterlerin seçimi klinikte hayat kurtarıcı bir öneme sahiptir.

A 68-year-old female patient with a congenital coronary artery anomaly (CCAA) presented to the emergency department with chest pain and elevated troponin levels indicative of acute coronary syndrome. During coronary angiography (CAG), the left circumflex artery (LCX) was absent in its normal location, and further evaluation revealed that the LCX originated anomalously from the right coronary artery (RCA). Due to its long-tip anatomical design, the initially utilized Judkins Right (JR) catheter occluded the LCX ostium, preventing the visualization of the culprit lesion. Switching to an Amplatz Left 1 (AL 1) catheter with a smaller tip successfully revealed a 100% occlusion in the proximal segment of the LCX, which was subsequently treated with balloon angioplasty and stenting. Post-procedural coronary CT angiography confirmed that the anomalous LCX followed a benign retroaortic course. While often incidental, CCAAs represent a significant cause of non-traumatic sudden cardiac death in young adults and pose diagnostic challenges during CAG. This presentation emphasizes that identifying missing coronary fields, recognizing catheter-induced misinterpretations, and swiftly selecting alternative catheters are critical, life-saving measures in clinical interventions.
