Özet

Renal arter anevrizması (RAA), visseral tip anevrizmaların yaklaşık dörtte birini oluşturan ve toplumda genel olarak %0,1 oranında rastlanan nadir bir vasküler patolojidir. Poutasse sınıflamasına göre sakküler, fuziform, disseke ve intrarenal olmak üzere dört temel tipe ayrılan bu hastalığa en sık sakküler (%70) formunda rastlanır. Kadınlarda ve fibromüsküler displazi hastalarında görülme sıklığı daha yüksek olan RAA; rüptür riski genellikle düşük olsa da gebelik, enflamatuar hastalıklar ve 3 cm’den büyük çap varlığında yüksek yırtılma riski taşır. Semptomatik yüksek tansiyon, ağrı, rüptür veya 2 cm'den büyük çap gibi durumlarda açık cerrahi, laparoskopi ya da endovasküler yöntemlerle müdahale edilmesi gerekir. Bu vaka sunumunda, 13 yıldır hipertansiyon hastası olan 38 yaşındaki bir kadında saptanan sağ renal arter orta kesimindeki 1,6 x 1,8 cm'lik sakküler anevrizmanın, stent destekli koil embolizasyonu (remodeling teknik) ile başarıyla tedavi edildiği gösterilmiştir. Minimal invaziv bu yöntem sayesinde, 3 aylık kontrol bilgisayarlı tomografisinde anevrizmanın tamamen elimine edildiği, sağ böbrek kan akımının başarıyla korunduğu ve hastanın antihipertansif ilaçlar olmaksızın taburcu edildiği bildirilmiştir.

Renal artery aneurysm (RAA) is a rare vascular disease comprising approximately one-fourth of visceral aneurysms, with an estimated general population prevalence of 0.1%. According to the Poutasse classification, it is categorized into four primary types: saccular, fusiform, dissected, and intrarenal, with the saccular form being the most common at 70%. RAA is more prevalent in females and individuals with fibromuscular dysplasia, and while the overall risk of rupture is relatively low, it increases during pregnancy, in the presence of inflammatory diseases, or when the diameter exceeds 3 cm. Intervention through open surgery, laparoscopy, or endovascular techniques is indicated in cases of rupture, symptomatic hypertension, pain, or a diameter greater than 2 cm. In this case report, a 1.6 x 1.8 cm saccular aneurysm in the middle section of the right main renal artery of a 38-year-old female with a 13-year history of hypertension was successfully treated using stent-assisted coil embolization (remodeling technique). This minimally invasive approach successfully eliminated the aneurysm, preserved right renal blood flow, and allowed the patient to be discharged with normal blood pressure without the need for antihypertensive medications at the 3-month follow-up.
