Koroner Bypass Operasyonu Sonrasında Sol İnternal Mamalian Grefti İle Pulmoner Arter Arasında Gelişen Fistül Vakasına Yaklaşım

Özet

Sol ön inen arter (LAD) bypass grefti olarak sol internal mamalian arter (LIMA) kullanımı, uzun dönem açıklık oranı nedeniyle yaygın olarak tercih edilmektedir. Koroner arter bypass greftlemesinden (KABG) sonra LIMA ile pulmoner arter (PA) arasında fistül gelişimi oldukça nadir bir klinik durumdur ve literatürde 30'dan az olgu bildirilmiştir. Bu olguda, 9 yıl önce KABG öyküsü olan ve kararsız angina pektoris semptomları ile polikliniğe başvuran 58 yaşında bir erkek hasta sunulmuştur. Yapılan koroner anjiyografide, LIMA ile akciğer sol lobuna giden PA arasında çok sayıda fistül saptanmış, aynı zamanda sirkumfleks arterin birinci optus marjininde %90 darlık izlenmiştir. Anjiyografi sırasında darlık olan bölgeye balon anjiyoplasti ve stent uygulaması yapılmış, işlem sonrasında hastanın göğüs ağrısı geçmiş ve elektrokardiyografideki ST depresyonları düzelmiştir. Operasyon sonrasında hastaya uygulanan miyokard perfüzyon sintigrafisinde iskemi saptanmaması ve efor anginasının olmaması nedeniyle, fistüle yönelik herhangi bir kapatma işlemi planlanmamış ve medikal izlem kararı alınmıştır. Beş yıllık takip sürecinde semptomları tekrarlamayan hasta, asemptomatik koroner fistül vakalarında konservatif ve medikal tedavinin öncelikli ve güvenli bir yaklaşım olabileceğini göstermektedir.

The utilization of the left internal mammary artery (LIMA) as a bypass graft for the left anterior descending artery (LAD) is widely preferred due to its long-term patency rates. The development of a fistula between the LIMA and the pulmonary artery (PA) after coronary artery bypass grafting (CABG) is an extremely rare clinical condition, with fewer than 30 cases reported in the literature. This case presents a 58-year-old male patient with a history of CABG 9 years ago who admitted to the outpatient clinic with symptoms of unstable angina pectoris. Coronary angiography revealed multiple fistulas between the LIMA and the PA leading to the left lobe of the lung, along with a 90% stenosis in the first obtuse marginal branch of the circumflex artery. Balloon angioplasty and stent deployment were performed on the stenotic segment during angiography, which resolved the chest pain and improved the ST segment depressions on the electrocardiography. Since myocardial perfusion scintigraphy showed no ischemia and the patient had no exertional angina post-procedure, no closure intervention was planned for the fistula, and medical management was chosen. The patient remained symptom-free during a five-year follow-up, demonstrating that conservative and medical treatment can be a primary and safe approach in asymptomatic coronary fistula cases.
