Marijuana’nın Neden Olduğu Perimiyokardit
Özet
Bu vaka sunumunda, bilinen bir kronik hastalığı olmayan ve göğüs ağrısı şikayetiyle başvuran 23 yaşında bir erkek hastada marijuana kullanımına bağlı gelişen perimiyokardit olgusu tartışılmaktadır. Hastanın özgeçmişinde son bir yıldır aralıklı olarak marijuana kullanma öyküsü mevcuttur. Yapılan elektrokardiyografide (EKG) ST segment elevasyonu ve PR segment depresyonu izlenirken, laboratuvar tetkiklerinde troponin ve CRP değerlerinde yükseklik saptanmıştır. Olası bir miyokard enfarktüsünü dışlamak amacıyla uygulanan koroner anjiyografide koroner arterlerin normal olduğu görülmüştür. Viral ve bakteriyel nedenlerin dışlanması ve idrar toksikoloji testinde δ-9 tetrahidrokanabinol (THC) pozitif saptanması üzerine hastaya marijuana kaynaklı perimiyokardit tanısı konulmuştur. Anjiyotensin dönüştürücü enzim inhibitörü, ibuprofen ve kolşisin tedavisi başlanan hastanın troponin değerlerinin düşmesi üzerine, yatışının ikinci gününde şifa ile taburcu edildiği belirtilmiştir. Sonuç olarak, esrar ve sentetik analoglarının kullanımı ciddi kardiyovasküler yan etkilere yol açabileceğinden, klinisyenlerin göğüs ağrısı ile başvuran genç hastalarda bu olasılığı göz önünde bulundurması ve tedavide marijuanadan kaçınmanın önemini vurgulaması gerekmektedir.
This case report discusses a case of perimyocarditis associated with marijuana use in a 23-year-old male patient with no known chronic diseases who presented with chest pain. The patient had a history of intermittent marijuana use for the past year. While electrocardiography (ECG) revealed ST-segment elevation and PR-segment depression, laboratory tests showed elevated troponin and CRP levels. Coronary angiography performed to rule out a potential myocardial infarction demonstrated normal coronary arteries. Following the exclusion of viral and bacterial etiologies and a positive urine toxicology screen for δ-9 tetrahydrocannabinol (THC), the patient was diagnosed with marijuana-induced perimyocarditis. Treatment with an angiotensin-converting enzyme inhibitor, ibuprofen, and colchicine was initiated, and the patient was discharged after two days of hospitalization upon the regression of troponin levels. Consequently, since cannabis and its synthetic analogs can trigger severe cardiovascular adverse events, clinicians must consider this possibility in young patients presenting with chest pain, and emphasize the absolute necessity of avoiding active and passive marijuana exposure during management.
