Lokalize Prostat Kanserine Yaklaşım

Özet

Dünya genelinde erkeklerde en sık görülen ikinci kanser türü olan prostat kanserine ve özellikle lokalize prostat kanserine yönelik güncel yaklaşım ve tedavi yöntemleri makalede detaylıca ele alınmaktadır. Tedavi planlamasında ilk adım evrelemedir ve hastalık lokalize (T1/2/erken T3, N0, M0), lokal ileri veya metastatik olarak sınıflandırılır. Yavaş ilerleme eğiliminde olan lokalize prostat kanserinde, tedavilerin yaşam kalitesine etkileri göz önünde bulundurularak hasta komorbiditeleri ve yaşam beklentisi değerlendirilmelidir. Standart yaklaşımlar arasında izlem tedavileri, radikal cerrahi, radyasyon tedavisi ve odak tedavileri yer alır. İzlem stratejilerinden "Bekle-Gör", semptomatik durumlarda tedaviyi hedeflerken; "Aktif İzlem" ise belirli kriterleri karşılayan hastalarda aşırı tedaviyi önlemek amacıyla serum PSA, mp-MRI ve tekrarlayan biyopsilerle yakın takibi içerir. Cerrahi tarafta radikal prostatektomi açık, laparoskopik veya robot yardımlı uygulanabilmekte, ancak yöntemlerin birbirine üstünlüğüne dair kesin kanıt bulunmamaktadır. Radyoterapi seçeneğinde çevre doku hasarını azaltan IMRT tercih edilirken, brakiterapi de düşük-orta riskli hastalarda doğrudan prostat içine radyoaktif madde yerleştirilerek uygulanır. Son olarak, fokal lazer ablasyonu ve HIFU gibi yenilikçi fokal tedaviler, yan etkileri azaltarak yaşam kalitesini korumayı amaçlasa da henüz klinik araştırma aşamasındadır.

The article thoroughly examines current approaches and treatment methods for localized prostate cancer, which is the second most commonly diagnosed cancer in men globally. Staging is the initial step in treatment planning, classifying the disease as localized (T1/2/early T3, N0, M0), locally advanced, or metastatic. Since localized prostate cancer tends to progress slowly, patient comorbidities and life expectancy must be evaluated, considering the impact of treatments on the quality of life. Standard approaches include surveillance therapies, radical surgery, radiation therapy, and focal treatments. Among surveillance strategies, "Watchful Waiting" targets deferred or symptomatic treatment, whereas "Active Surveillance" involves close monitoring via serum PSA, mp-MRI, and repeat biopsies to prevent overtreatment in eligible patients. Regarding surgery, radical prostatectomy can be performed open, laparoscopically, or robot-assisted, though evidence demonstrating the superiority of any specific method is lacking. In radiotherapy, IMRT is preferred to reduce peripheral tissue toxicity, while brachytherapy is utilized for low-to-medium risk patients by placing radioactive sources directly into the prostate. Lastly, innovative focal treatments like focal laser ablation and HIFU aim to preserve quality of life by minimizing side effects, but they are currently limited to clinical trials.
