Penil Kanserlere Yaklaşım
Özet
Penis kanseri, Human Papilloma Virus (HPV) ve fimozis gibi temel risk faktörleriyle ilişkili, nadir görülen agresif bir malignitedir. En yaygın histolojik tip olan skuamöz hücreli karsinomun (SCC) klinik bulguları heterojen olup, kesin tanı penisteki lezyonun histopatolojik incelemesiyle konur. Evrelemede TNM sınıflandırması tercih edilirken; primer tümörün invazyon derinliği, inguinal lenf nodlarının durumu ve uzak metastaz varlığı prognozu belirleyen en kritik unsurlardır. Tedavi stratejileri klinik evreye göre şekillenir ve onkolojik prensiplerden ödün vermeden organ koruyucu yaklaşımlar (lazer, topikal kemoterapi, parsiyel cerrahi, radyoterapi) ön planda tutulur; ileri evrelerde ise radikal cerrahi ve kemoterapi uygulanır. Nükslerin erken tespiti ve hastalıksız sağ kalım için primer tümör ve lenf nodlarının en az 5 yıl boyunca düzenli takibi hayati önem taşımaktadır.
Penile cancer is a rare yet aggressive malignancy closely associated with key risk factors such as Human Papilloma Virus (HPV) and phimosis. The clinical presentation of squamous cell carcinoma (SCC), which is the most common histological type, is highly heterogeneous, and a definitive diagnosis relies entirely on the histopathological evaluation of the penile lesion. While the TNM classification system is utilized for staging, the depth of invasion of the primary tumor, the status of the inguinal lymph nodes, and the presence of distant metastases constitute the most critical prognostic factors. Treatment strategies are tailored according to the clinical stage, prioritizing organ-preserving approaches (such as laser therapies, topical chemotherapy, partial surgery, and radiotherapy) without compromising oncological principles, whereas radical surgery and systemic chemotherapy are mandatory in advanced stages. Regular and meticulous follow-up of both the primary tumor site and the lymph nodes for at least 5 years is of vital importance for the early detection of recurrences and for ensuring long-term disease-free survival.
