Özet

Fıtık, bir organ veya dokunun anormal bir açıklıktan dışarı çıkmasıdır. Tarihsel süreçte fıtık tedavisi, ameliyatsız yaklaşımlardan günümüzün minimal invaziv cerrahi yöntemlerine kadar büyük bir evrim geçirmiştir. İlk yazılı tıbbi kayıt olan Ebers Papirüsü'nde fıtık ameliyatından bahsedilse de, eski çağlarda anestezi ve asepsi eksikliği nedeniyle cerrahi girişimler oldukça riskli ve ölümcüldü. 19. yüzyılın başlarında kasık anatomisinin netleşmesi ve sterilizasyon tekniklerinin gelişmesiyle cerrahi onarımlar hız kazanmıştır. 1871'de Henry O. Marcy iç kasık halkasını ilk kez süture ederek kapatmış, ardından Bassini modern fıtık tamirinin temelini oluşturan üç katlı onarım yöntemini geliştirmiştir. Cerrah Billroth’un güçlü suni doku arayışı vizyonu doğrultusunda, 1950'lerde Usher polipropilen yama kullanımını başlatarak fıtık cerrahisinde çığır açmıştır. 1980'lerde Lichtenstein, dokuyu germeden yama ile onaran "gerilimsiz fıtık" kavramını popülerleştirmiştir. Günümüzde ise ilk kez Ger tarafından tarif edilen laparoskopik ve minimal invaziv yaklaşımlar (TAPP, TEP), ameliyat sonrası ağrıyı azaltarak ve iyileşmeyi hızlandırarak modern tedavi standardını oluşturmaktadır. Tüm bu tarihsel süreç, başarılı bir fıtık tedavisinin nihai olarak iyi bir anatomi bilgisine dayandığını göstermektedir.

A hernia is defined as the protrusion of an organ or tissue through an abnormal opening. Throughout history, hernia treatment has evolved significantly from non-surgical reduction methods using belts or corsets to modern minimal invasive surgical techniques. Although hernia surgery was first mentioned in the ancient Egyptian Ebers Papyrus, early surgical interventions were rare, extremely painful, and associated with high mortality rates due to the lack of anesthesia and asepsis. In the early 19th century, a detailed understanding of groin anatomy and the development of sterilization techniques paved the way for new surgical methods. Henry O. Marcy became the first surgeon to suture and close the internal inguinal ring in 1871, but Bassini is widely accepted as the inventor of modern hernia repair with his three-layer anatomical reconstruction. Driven by Billroth's vision of utilizing synthetic materials as strong as natural tissues, Usher introduced the use of polypropylene mesh in the 1950s, leading to significant advancements. In the 1980s, Lichtenstein revolutionized the field by introducing the tension-free hernia repair concept using mesh, which surpassed older techniques. Today, laparoscopic approaches such as TAPP and TEP, initially described by Ger, represent the current standard by minimizing postoperative pain and facilitating faster wound healing, while successful treatment ultimately relies on a comprehensive knowledge of anatomy.
