Kasık Bölgesi Anatomisi
Özet
Kasık anatomisi, fıtık onarım cerrahisinde nüks ve komplikasyonları en aza indirmek için hayati öneme sahiptir. Bölgedeki temel kanallar inguinal ve femoral kanallardır. İnguinal kanal, inguinal ligamanın üstünde yer alır ve içinden erkeklerde spermatik kord, kadınlarda ise rotundum ligamanı geçer. Kanalın ön duvarını eksternal oblik aponevrozu, arka duvarını ise fasya transversalis oluşturur. Hasselbach üçgeninin içinden çıkan fıtıklar direkt, lateralinden çıkanlar ise indirekt fıtık olarak tiplendirilir. Femoral kanal ise inguinal ligamanın altında bulunur; anteriorunda iliopubik trakt, posteriorunda Cooper ligamanı ve lateralinde femoral ven yer alır. Çocuklarda bu kanal yetişkinlere göre daha kısa olup katmanlar farklılık gösterebilir. Bölgedeki iliohipogastrik, ilioinguinal ve genitofemoral sinirler duyusal uyarımı sağlarken, preperitoneal alandaki vasküler yapılar laparoskopik cerrahide kritik referans noktalarıdır. Kasık lenfatikleri ise yüzeyel, derin (Cloquet nodülü dahil) ve aberran olarak üç grupta incelenir ve alt ekstremite ile perine bölgesinin drenajını üstlenir.
Groin anatomy is vital for minimizing recurrence and complications in hernia repair surgery. The primary channels in this region are the inguinal and femoral canals. The inguinal canal is located above the inguinal ligament and contains the spermatik cord in men and the round ligament in women. Its anterior wall is formed by the external oblique aponeurosis, while the posterior wall is formed by the transversalis fascia. Hernias arising within the Hesselbach triangle are classified as direct, whereas those emerging laterally are indirect. The femoral canal lies below the inguinal ligament, bordered anteriorly by the iliopubic tract, posteriorly by the Cooper ligament, and laterally by the femoral vein. In infants, this canal is shorter, and facial layers may differ from adults. The iliohypogastric, ilioinguinal, and genitofemoral nerves provide sensory innervation, while vascular structures in the preperitoneal space serve as critical landmarks during laparoscopic surgery. Groin lymphatics are categorized into superficial, deep (including Cloquet's node), and aberrant groups, draining the lower extremity and perineal regions.
