Özet

Femoral herni, tüm kasık fıtıklarının %4’ünü oluşturan, nadir fakat yüksek inkarserasyon ve strangülasyon riski (%35) taşıyan bir fıtık türüdür. İleri yaşlarda ve pelvis yapısının genişliği nedeniyle kadınlarda daha sık görülür. Diğer fıtıklara göre ölüm oranı 10 kat fazla olduğundan, femoral hernilerde izlem önerilmez ve acil veya erken elektif cerrahi şarttır. Anatomik olarak femoral kanal, femoral kılıfın medialinde yer alır ve karın duvarındaki zayıf noktalardan biridir. Preoperatif dönemde özellikle obez hastalarda teşhis zorluğu yaşanabilir; bilgisayarlı tomografi cerrahi planlamayı kolaylaştırır. Tedavide infrainguinal, transinguinal ve açık retroperitoneal yaklaşımların yanı sıra laparoskopik (TAPP, TEP) yöntemler de tercih edilmektedir. Cerrahi işlemlerde dikişle yapılan primer onarımlar yüksek nüks ve gerginliğe bağlı ağrı riski taşırken, mesh (yama) veya mesh plug (tıkaç) kullanımı nüks oranlarını düşürmesi bakımından üstündür. Ameliyat sonrasında hematom, seroma ve enfeksiyon gibi komplikasyonlar gelişebilir; büyük defektlerde aspiratif diren kullanımı ve erken mobilizasyon önerilir. Sonuç olarak, hastanın durumu ve cerrahın tecrübesine göre seçilen doğru yaklaşım ve kontrendikasyon olmadığı sürece yama kullanımı, cerrahi başarıyı ve hasta sağkalımını optimize eden en temel unsurlardır.

Femoral hernia is a rare type of hernia accounting for 4% of all groin hernias, but it carries a high risk of incarceration and strangulation (35%). It is more common in older ages and in women due to the width of the female pelvis. Since its mortality rate is 10 times higher compared to other hernia types, watchful waiting is not recommended, and early elective or urgent surgery is mandatory. Anatomically, the femoral canal is located medially within the femoral sheath and constitutes a weak point in the abdominal wall. Preoperatively, diagnostic difficulties may arise particularly in obese patients, whereas computed tomography facilitates surgical planning. Regarding treatment, infrainguinal, transinguinal, and open retroperitoneal approaches, as well as laparoscopic (TAPP, TEP) methods, are utilized. In surgical procedures, primary suture repairs carry a high risk of recurrence and tension-induced pain, while the use of a mesh or mesh plug is superior in reducing recurrence rates. Postoperative complications such as hematoma, seroma, and infection may occur; therefore, the use of a suction drain in large defects and early mobilization are recommended. Consequently, choosing the correct approach based on the patient's condition and the surgeon's experience, combined with mesh placement unless contraindicated, represents the most fundamental element that optimizes surgical success and patient survival.
