Özet

Karın duvarı; cilt, kaslar ve fasyalardan oluşan, iç organları koruyan mezoderm kökenli bir yapıdır. Ön orta hatta rektus abdominis kaslarının birleştiği kılıfa linea alba adı verilir ve cerrahi giriş noktası olduğundan fıtıklar en sık burada görülür. Bölgenin anatomisinde eksternal oblik, internal oblik ve transversus abdominis kasları ile posterior rektus kılıfının desteksiz kaldığı arkuat hat kritik öneme sahiptir. Abdominal ön duvar hernileri herediter veya sonradan oluşabilir; en sık semptomu ıkınmayla büyüyen kitledir. Redükte edilemeyen fıtıklar "inkarsere" olarak adlandırılır ve acil cerrahi gerektirir; kanlanma bozulursa "strangüle herni" gelişir. Fıtıklar lokalizasyonlarına göre epigastrik, umblikal ve Spiegel hernileri olarak sınıflandırılır. Yenidoğanların %10'unda görülen umblikal herniler genellikle kendiliğinden kapanırken, erişkinlerde 2 cm'den büyük veya semptomatik defektlerde cerrahi ve yama kullanımı gerekir. Assit ve umblikal herni birlikteliğinde, karın içi basıncı artıran assit tedavi edilmeden cerrahiye geçilmemelidir; tedavisinde parasentez, TIPS veya karaciğer transplantasyonu uygulanır. Linea semilunaris üzerinde nadiren görülen Spiegel hernileri ise %17 gibi yüksek bir inkarserasyon riskine sahip olduğundan teşhis edildiği an açık veya laparoskopik olarak ameliyat edilmelidir.

The abdominal wall is a mesoderm-originated structure consisting of skin, muscles, and fasciae that protects internal organs, with hernias most frequently occurring along the linea alba, the midline where rectus abdominis muscles unite, due to its common use as a surgical entry point. The anatomy is defined by the external oblique, internal oblique, and transversus abdominis muscles, alongside the arcuate line below which the posterior rectus sheath lacks support. Abdominal anterior wall hernias, presenting as hereditary or acquired defects often characterized by a swelling that enlarges with straining, require urgent surgery if they become irreducible (incarcerated), and can progress to strangulated hernias if the blood supply is compromised. Classified by localization, epigastric hernias occur between the xiphoid and umbilicus; umbilical hernias, prevalent in 10% of newborns, mostly resolve spontaneously but require surgical repair with mesh in adults if the defect exceeds 2 cm or becomes symptomatic. In cases where ascites coexists with an umbilical hernia, the increased intraabdominal pressure must be managed via paracentesis, TIPS, or liver transplantation before performing hernia surgery. Lastly, Spigelian hernias, which develop rarely along the linea semilunaris and most commonly at the level of the arcuate line, necessitate immediate open or laparoscopic surgical correction upon diagnosis due to a high incarceration risk of 17%.
