Özet

Laparoskopik Transabdominal Preperitoneal (TAPP) kasık fıtığı onarımı, yüksek anatomik bilgi ve teknik beceri gerektiren etkin bir cerrahi yöntemdir. Makalede, ameliyatın güvenle uygulanabilmesi için Kıyamet (doom) ve Ağrı (pain) üçgenleri gibi hayati vasküler ve sinirsel yapıların yer aldığı preperitoneal anatomiyi derinlemesine tanımanın önemi vurgulanmaktadır. Tanı aşamasında genellikle fizik muayene yeterli olmakla birlikte, karmaşık vakalarda ultrason, BT ve MR gibi görüntüleme yöntemlerine veya laparoskopik eksplorasyona başvurulmaktadır. TAPP yöntemi özellikle nükseden ve çift taraflı fıtıklarda tercih edilirken; kapnoperitoneumu tolere edemeyen hastalar, sepsis tablosu, ciddi asit varlığı ve 15 yaş altı çocuklar gibi durumlar ameliyat için kontrendikasyon oluşturur. Ameliyat sürecinde Hasson tekniği ile trokar yerleşimi, peritoneal flep diseksiyonu, fıtık kesesinin küçültülmesi, uygun boyutta (12-15 cm x 11-13 cm) polipropilen yama yerleştirilmesi ve Cooper ligamanına sabitlenerek peritonun kapatılması adımları sırasıyla uygulanır. Cerrahi sonrası hastalar genellikle 24 saat içinde taburcu edilir ve hafif aktivitelere yönlendirilir. Ameliyat esnasında epigastrik veya korona mortis damarlarında kanama; ameliyat sonrasında ise seroma, hematom ve sabitleme klipslerine bağlı kronik nöropatik ağrılar en sık karşılaşılan komplikasyonlar arasındadır. Sonuç olarak, TAPP yönteminin başarısı cerrahın anatomi hakimiyeti ve bimanüel diseksiyon deneyimine doğrudan bağlıdır.

Laparoscopic Transabdominal Preperitoneal (TAPP) inguinal hernia repair is an effective surgical method that requires high anatomical knowledge and technical skill. The article emphasizes the importance of deeply understanding the preperitoneal anatomy, which includes vital vascular and neural structures like the triangles of doom and pain, to safely perform the procedure. While physical examination is generally sufficient for diagnosis, imaging modalities such as USG, CT, and MRI, or laparoscopic exploration, are utilized in complex cases. The TAPP method is particularly preferred for recurrent and bilateral hernias; however, conditions like intolerance to capnoperitoneum, sepsis, severe ascites, and pediatric patients under 15 years old pose contraindications. The surgical process sequentially involves trocar placement via the Hasson technique, peritoneal flap dissection, reduction of the hernia sac, placement of an appropriately sized (12-15 cm x 11-13 cm) polypropylene mesh, fixation to Cooper's ligament, and closure of the peritoneum. Postoperatively, patients are typically discharged within 24 hours and guided toward light activities. Intraoperative bleeding from epigastric or corona mortis vessels, and postoperative complications such as seromas, hematomas, and chronic neuropathic pain due to fixation clips, are the most common adverse events. In conclusion, the success of the TAPP method is directly dependent on the surgeon's mastery of anatomy and bimanual dissection experience.
