Total Ekstraperitoneal İnguinal Herni Onarımı (TEP), laparoskopik kasık fıtığı onarımında kullanılan ve batın içine girilmeden preperitoneal alana sentetik yama yerleştirilmesini sağlayan etkin bir cerrahi tekniktir. İlk olarak 1993 yılında başarılı şekilde uygulanan bu yöntem, geleneksel açık onarımlara kıyasla daha az ameliyat sonrası ağrı, daha kısa hastanede yatış süresi, daha az hematom ve derin yara yeri enfeksiyonu gibi önemli avantajlar sunmaktadır. TEP, miyopektineal orifise posterior yaklaşım sağlayarak inguinal, femoral ve obturator hernilerin aynı anda onarılmasına imkan tanır. Tekniğin başarısı, "kıyamet üçgeni" ve "ağrı üçgeni" gibi kritik nörovasküler yapıları barındıran preperitoneal boşluk anatomisinin çok iyi bilinmesine bağlıdır. Genellikle bilateral ve nüks fıtıklarda, ayrıca fiziksel aktiviteye erken dönmek isteyen hastalarda tercih edilen TEP; lokal veya sistemik enfeksiyonlar ile geçirilmiş alt batın ameliyatlarına bağlı ciddi skar dokusu varlığında kontrendikedir. Ameliyat sırasında anatomik yapıların korunması, peritoneal yırtıkların yönetilmesi ve en az 12x15 cm boyutlarında uygun polipropilen yamanın fiksasyonu nüksü ve kronik ağrıyı önlemede kritik adımlardır. Öğrenme eğrisi zor olmakla birlikte, TEP tekniği nüks ve komplikasyon oranları açısından diğer yöntemlerle eşdeğer sonuçlara sahiptir.

Total Extraperitoneal Inguinal Hernia Repair (TEP) is an effective surgical technique used in laparoscopic groin hernia repair that allows the placement of a synthetic mesh into the preperitoneal space without entering the abdominal cavity. First successfully performed in 1993, this method offers significant advantages compared to traditional open repairs, such as less postoperative pain, shorter hospital stay, fewer hematomas, and a lower rate of deep wound infections. TEP approaches the myopectineal orifice posteriorly, enabling the simultaneous repair of inguinal, femoral, and obturator hernias. The success of the technique depends heavily on a thorough understanding of the preperitoneal space anatomy, which contains critical neurovascular structures like the "triangle of doom" and the "triangle of pain". TEP is generally preferred for bilateral and recurrent hernias, as well as for patients seeking an early return to physical activity; however, it is contraindicated in the presence of local or systemic infections and severe scar tissue due to previous lower abdominal surgeries. Protecting anatomical structures during surgery, managing peritoneal tears, and fixing an appropriate polypropylene mesh of at least 12x15 cm are critical steps to prevent recurrence and chronic pain. Although its learning curve is challenging, the TEP technique has comparable results to other methods regarding recurrence and complication rates.
