Neoadjuvan Tedavi Prensipleri
Özet
Neoadjuvan tedavi, lokal tedavi yöntemlerinden (genellikle cerrahi) önce uygulanan sistemik ilaç ve radyoterapi süreçlerini kapsayarak modern onkolojide kritik bir rol oynamaktadır. Bu tedavi modelinin temel amacı, tümör hacmini küçülterek başlangıçta opere edilemeyen hastaları cerrahiye uygun hale getirmek, organ fonksiyonlarını korumak ve mikrometastazları erken evrede elimine etmektir. Tümörün tedaviye verdiği yanıtın doğrudan gözlemlenmesi, klinik uzmanlara bir in vivo ilaç duyarlılık testi imkanı sunarak sonraki tedavi adımlarının optimize edilmesine olanak tanır. Klinik araştırmalar anal kanal, rektum, mesane, baş-boyun ve meme kanserleri ile osteosarkom gibi agresif malignitelerde neoadjuvan yaklaşımların sağkalım sürelerini uzattığını ve lokal nüks riskini bariz şekilde düşürdüğünü ortaya koymaktadır. Özellikle anal kanal kanserinde sfinkter fonksiyonlarının korunmasını sağlayarak hastaların yaşam kalitesini artırması, bu yöntemin en somut başarılarındandır.
Neoadjuvant therapy encompasses systemic drug regimens and radiotherapy administered prior to definitive local interventions like surgery, playing a pivotal role in modern oncology. The primary objective of this therapeutic model is to downstage tumor volume, converting unresectable cases into surgically manageable ones, while preserving organ functions and eliminating early micrometastases. Direct observation of the tumor's response provides clinical specialists with an invaluable in vivo drug sensitivity test, allowing for the precise optimization of subsequent therapeutic steps. Clinical research indicates that neoadjuvant approaches significantly prolong survival times and reduce the risk of local recurrence in aggressive malignancies such as anal, rectal, bladder, breast, head and neck cancers, and osteosarcoma. Notably, its capacity to preserve sphincter function in anal canal cancer, thereby enhancing the patient's post-treatment quality of life, represents one of the most substantial milestones of this preoperative methodology.
