Özet

Hücreler, dış streslere ve fizyolojik hasarlara karşı homeostazı korumak adına belirli adaptasyon mekanizmaları geliştirir; ancak uyum yeteneklerini aşan ağır ve uzun süreli etkenlerle karşılaştıklarında geri dönüşsüz hasara uğrayarak nekroz veya apoptoz yoluyla ölürler. Hücre zedelenmesinin temel nedenleri arasında hipoksi ve iskemi, serbest radikallerin yol açtığı oksidatif stres, iskemi-reperfüzyon hasarı, endoplazmik retikulum stresi, kimyasal toksinler, DNA hasarı ve inflamasyon yer almaktadır. Hücre ölümü tiplerinden nekroz; hücre zar yapısının bozulması, hücresel enzimlerin dışarı sızması ve komşu dokuda sıklıkla inflamasyon oluşması ile karakterize olan patolojik bir süreçtir. Koagülasyon, likefaksiyon, gangrenöz, kazeifikasyon, yağ ve fibrinoid nekrozu gibi farklı morfolojik tipleri bulunur. Programlanmış hücre ölümü olan apoptozis ise genellikle fizyolojik bir süreç olup, hücre bütünlüğünü koruyarak komşu dokularda inflamasyona yol açmadan mitokondri veya ölüm reseptörü yolakları üzerinden gerçekleşir. Hücrelerin çevresel değişikliklere yanıt olarak gösterdiği uyum biçimleri ise sayıca artış (hiperplazi), boyutsal büyüme (hipertrofi), küçülme (atrofi) ve hücre tipinin değişmesi (metaplazi) şeklinde ortaya çıkan hücresel adaptasyon şekilleridir.

Cells develop specific adaptation mechanisms to maintain homeostasis against external stresses and physiological damages; however, when faced with severe and prolonged factors exceeding their adaptive capacity, they undergo irreversible damage, leading to cell death via necrosis or apoptosis. The primary causes of cell injury include hypoxia and ischemia, oxidative stress caused by free radicals, ischemia-reperfusion injury, endoplasmic reticulum stress, chemical toxins, DNA damage, and inflammation. Among the types of cell death, necrosis is a pathological process characterized by the disruption of the cell membrane structure, leakage of cellular enzymes, and frequent inflammation in adjacent tissues. It has different morphological types such as coagulation, liquefactive, gangrenous, caseous, fat, and fibrinoid necrosis. Apoptosis, which is programmed cell death, is generally a physiological process and occurs through mitochondrial or death receptor pathways by preserving cellular integrity without causing inflammation in adjacent tissues. The ways cells adapt in response to environmental changes are cellular adaptation forms that manifest as an increase in number (hyperplasia), dimensional growth (hypertrophy), shrinkage (atrophy), and alteration of cell type (metaplasia).
