Özet

Yara iyileşmesi ve onarımı; hemostaz, inflamasyon, proliferasyon ve yeniden şekillenme (remodelling) aşamalarından oluşan, hücreler ve inflamatuar mediatörler arasındaki kompleks etkileşimleri içeren karmaşık bir süreçtir. Dokunun zedelenmesiyle başlayan hemostaz evresinde, plateletler devreye girerek ilk fibrin tıkacını oluşturur ve kanamayı kontrol altına alır. Ardından gelen inflamasyon evresinde, lökositler yara bölgesine göç ederek fagositoz yapar, dokuyu mikroorganizmalara karşı korur ve temizler. Proliferasyon fazında ise fibroblastlar ve endotel hücreleri aktif rol oynar; yeni kan damarlarının oluşumu (anjiogenezis) ve ekstraselüller matriks sentezi ile parlak kırmızı renkli granülasyon dokusu oluşturulur. En uzun süren yeniden şekillenme evresinde ise Tip III kollajen yerini Tip I kollajene bırakarak dokunun gerilme direncini artırır ve süreç skar oluşumuyla tamamlanır. Bu süreçlerin koordinasyonunda EGF, PDGF, FGF ve TGF-beta gibi polipeptid yapıdaki büyüme faktörleri ile sitokinler; hücre göçü, bölünmesi ve matürasyonunda kritik görevler üstlenir. Yara iyileşmesi; lokal olarak enfeksiyon, hematom, yabancı cisim ve radyoterapiden; sistemik olarak ise yaş, malnütrisyon, glukokortikoid kullanımı, diyabet ve üremi gibi faktörlerden doğrudan etkilenerek gecikebilir veya olumsuz yapılanabilir.

Wound healing and repair is a complex process involving intricate interactions between cells and inflammatory mediators, consisting of hemostasis, inflammation, proliferation, and remodeling phases. In the hemostasis phase, which begins with tissue injury, platelets intervene to form the initial fibrin plug and control bleeding. During the subsequent inflammation phase, leukocytes migrate to the wound area to perform phagocytosis, protecting and clearing the tissue against microorganisms. In the proliferation phase, fibroblasts and endothelial cells play an active role, forming bright red granulation tissue through new blood vessel formation (angiogenesis) and extracellular matrix synthesis. In the remodeling stage, which takes the longest time, Type III collagen is replaced by Type I collagen, increasing the tensile strength of the tissue, and the process concludes with scar formation. In the coordination of these processes, cytokines and polypeptide-structured growth factors such as EGF, PDGF, FGF, and TGF-beta undertake critical duties in cell migration, division, and maturation. Wound healing can be delayed or adversely affected locally by infection, hematoma, foreign bodies, and radiotherapy, and systemically by factors such as age, malnutrition, glucocorticoid use, diabetes, and uremia.
