Özet

Neoplazi, hücrelerin kontrolsüz çoğalmasıyla oluşan iyi (benign) veya kötü huylu (malign) büyümelerdir. Benign tümörler genellikle yavaş büyür, kapsüllüdür ve metastaz yapmazlar; malign tümörler (kanser) ise hızlı büyür, çevre dokuları istila eder ve uzak bölgelere yayılır (metastaz). Kanserler mikroskobik anaplazi derecesi ve büyüme hızına göre Broders sistemine göre I-IV arası derecelendirilir. Yayılım derecesi ise TNM (Tümör, Nodal tutulum, Metastaz) ve AJC evreleme sistemleriyle belirlenir. Kanserin ortaya çıkmasında; genetik yatkınlık (örneğin BRCA mutasyonları, Down sendromu), coğrafi-ırksal etkenler, yaş ve cinsiyet gibi endojen faktörlerin yanı sıra sigara, alkol, kötü beslenme, endüstriyel kimyasallar (asbest vb.), UV ışınları ve iyonlaştırıcı radyasyon gibi çevresel ajanlar rol oynar. Ayrıca HPV, HBV, HCV ve Helicobacter pylori gibi biyolojik ajanlar da viral/bakteriyel onkogenez yoluyla belirli kanserlerin gelişimine neden olur. Kimyasal karsinojenez; inisiyasyon (başlatma), promosyon (artma) ve progresyon (ilerleme) olmak üzere birbirini takip eden üç aşamalı, genetik mutasyonlarla ilerleyen çok basamaklı bir süreçtir.

Neoplasia is the uncontrolled proliferation of cells resulting in benign or malignant growths. Benign tumors generally grow slowly, are encapsulated, and do not metastasize; whereas malignant tumors (cancer) grow rapidly, invade adjacent tissues, and spread to distant sites via metastasis. Cancers are graded from I to IV using the Broders system based on their microscopic degree of anaplasia and growth rate. The extent of spread is evaluated using TNM (Tumor, Nodal involvement, Metastasis) and AJC staging systems. Cancer development is driven by endogenous factors such as genetic predisposition (e.g., BRCA mutations, Down syndrome), geographical-racial influences, age, and gender, along with environmental agents including smoking, alcohol, poor diet, industrial chemicals like asbestos, UV light, and ionizing radiation. Furthermore, biological agents such as HPV, HBV, HCV, and Helicobacter pylori cause specific cancers through viral or bacterial oncogenesis. Chemical carcinogenesis is a multi-step process involving sequential stages of initiation, promotion, and progression, which drives cellular transformation through progressive genetic mutations.
