Gövde ve Üst Ekstremitede Kullanılan Koruyucu Ortezler ve Destek Ekipmanları

Özet

Gövde ve üst ekstremite segmentlerinde sık karşılaşılan patolojik ve postüral değişikliklerin önlenmesinde kullanılan koruyucu ortezler ile destek ekipmanları, bozulan biyomekanik yapıya bağlı işlevsel kayıpları engellemede kritik bir role sahiptir. Profilaktik yaklaşımların öncelikli olduğu bu alanda gövde destekleri; mekanik bel ağrılarında akut dönemde önerilen elastik bel desteklerini, osteoporotik vertebral kırık riskini azaltan spinal ortezleri ve gebelikte sakroiliak stabilizasyon sağlayan pelvik kompresyon bantlarını kapsamaktadır. Üst sırt ve omuz bölgesinde, masa başı çalışanlarda sık görülen Upper Cross sendromuna karşı omuz retraksiyon ortezleri ve postür düzeltme giysileri öne çıkmaktadır. Baş ve boyun korumasında, travma sonrası immobilizasyon sağlayan Philadelphia boyunluk ile pediatrik grupta plagiosefali gibi deformasyonlarda kullanılan kişiye özel helmet uygulamaları incelenmektedir. Üst ekstremite destekleri kapsamında; dirsekte lateral epikondilit için geliştirilen epikondilit bantları, kübital tünel sendromunda fleksiyonu sınırlayan gece ortezleri, kış sporlarında kırıkları önleyen el bileği koruyucuları, Karpal Tünel Sendromu ortezleri ve De Quervain tenosinoviti splintleri detaylandırılmaktadır. Son olarak, başparmak CMC osteoartritinde kullanılan kısa oponens ortezleri ile kaya tırmanışçılarında fleksör tendon pulley yaralanmalarını önlemeye yönelik parmak bantlama yöntemleri açıklanmaktadır. Tüm bu koruyucu ekipmanların, bireylerin fonksiyonel kapasitesini kısıtlamayacak şekilde kişiselleştirilmesi ve uzmanlar eşliğinde primer tedaviye destekleyici olarak kullanılması gerekmektedir.

Protective orthoses and support equipment used to prevent common pathological and postural changes in the trunk and upper extremity segments play a critical role in preventing functional losses caused by impaired biomechanics. Emphasizing the priority of prophylactic approaches, the trunk supports section details elastic lumbar supports recommended for acute phases of mechanical low back pain, spinal orthoses reducing vertebral fracture risks in postmenopausal geriatric populations, and pelvic compression bands providing sacroiliac stabilization during pregnancy. For the upper back and shoulder, it analyzes shoulder retraction orthoses and posture-correcting garments against Upper Crossed syndrome. In head and neck protection, the focus is on Philadelphia collars mandatory for post-trauma immobilization and custom-made helmets used for pediatric conditions like plagiocephaly. Regarding upper extremity supports, it outlines epicondylitis bands for lateral epicondylitis, night splints restricting flexion for cubital tunnel syndrome, wrist guards preventing fractures in winter sports, Carpal Tunnel Syndrome orthoses, and De Quervain's tenosynovitis splints. Finally, short opponens orthoses for thumb CMC osteoarthritis and finger taping methods protecting flexor tendon pulleys in rock climbers are described. It concludes that these prophylactic devices must be customized to preserve functional capacity and used under healthcare professionals' guidance as adjunctive modalities to primary treatments.
