Ayak ve Alt Ekstremitede Kullanılan Koruyucu Ortezler ve Destek Ekipmanları
Özet
Alt ekstremite segmentleri, yaşam boyu maruz kaldıkları travma ve mekanik stresler nedeniyle korunmaya ihtiyaç duyar. Bu amaçla kullanılan koruyucu ortezler ve destek ekipmanları; ayak, ayak bileği, diz ve kalça eklemlerine yönelik geniş bir yelpazeyi kapsar. Ayakkabı seçimi, biyomekanik uyum ve fonksiyon kriterlerine dayanmalıdır; çünkü uygun olmayan ayakkabılar çocuklarda gelişimi engellerken, yaşlılarda düşme riskini ve diyabetli bireylerde ise ciddi ayak ülserlerini tetikleyebilir. Çocukluk döneminde esnek tabanlı ayakkabılar propriyoseptif duyuyu uyarırken, yaşlılarda denge için ince ve orta sertlikte tabanlar ile yüksek konçlu modeller önerilir. Diyabetik ayak yönetiminde ise plantar basıncı dağıtan kişiye özel ortezler ve rocker tabanlı botlar kritik öneme sahiptir. Ayrıca, alt ekstremite boy eşitsizliklerinde topuk kamaları, halluks valgusta destekleyici pedler ve spor yaralanmalarında mediolateral stabilite sağlayan ayak bileği ortezleri yaygın olarak kullanılır. Patellofemoral ağrı sendromunda dizilim bozukluklarını düzeltmek için neopren dizlikler tercih edilirken, yaşlı popülasyondaki düşmelerde kalça kırığı insidansını azaltmak adına koruyucu kalça dolguları ve kalkanları biyomekanik koruma sağlar. Tüm bu harici destekler, cerrahi olmayan konservatif tedavinin ve fizyoterapi uygulamalarının başarısını artıran tamamlayıcı unsurlardır.
Lower extremity segments require protection due to life-long exposure to trauma and mechanical stresses. Protective orthoses and support equipment used for this purpose encompass a wide range of devices targeting the foot, ankle, knee, and hip joints. Footwear selection must be based on biomechanical fit and functional criteria, as ill-fitting shoes can impair development in children, increase the risk of falls in the elderly, and trigger severe foot ulcers in individuals with diabetes. While flexible-soled shoes stimulate proprioceptive input during childhood, thin, moderately firm soles and high-top designs are recommended for balance in older adults. In diabetic foot management, custom orthoses and rocker-soled boots that redistribute plantar pressure are of critical importance. Additionally, heel wedges for leg length discrepancy, supportive pads for hallux valgus, and ankle orthoses providing mediolateral stability in sports injuries are widely utilized. Neoprene braces are preferred to correct maltracking in patellofemoral pain syndrome, whereas protective hip paddings and shields provide biomechanical protection to reduce the incidence of hip fractures from falls in the elderly population. All these external supports serve as complementary elements that enhance the success of non-surgical conservative treatment and physical therapy practices.
