Vajinal Sitolojide Görülen Hücre Tipleri
Özet
Vajinal sitolojide mevcut hücre tiplerini belirlemek için hazırlanan yayma örnekleri ışık mikroskobu altında incelenir ve optimum görüntüleme için Köhler aydınlatması kullanılır. İncelemeye düşük büyütmeyle başlanıp kornifiye skuamöz epitel hücreleri, lökositler ve çekirdekli epitel hücreleri ayırt edilerek ratın östrus siklusundaki aşaması belirlenir. Nötrofiller (lökositler) yuvarlak, çok küçük ve çok loblu çekirdeklere sahip hassas hücrelerdir; epitel hücrelerinden küçük boyutları ve düzensiz şekilleriyle ayrılırlar. Çekirdekli epitel hücreleri hafif boyalı sitoplazma ve oval çekirdeğe sahiptir. Küçük çekirdekli epitel hücreleri yuvarlak, keratinize olmayan ve yüksek nükleus:sitoplazma oranına sahip hücrelerdir. Büyük çekirdekli epitel hücreleri ise bol sitoplazmalı, düşük nükleus:sitoplazma oranına sahip, yuvarlak ila çokgen şekilli ve kısmen keratinize olabilen hücrelerdir. Skuam veya "kornifiye" olarak bilinen çekirdeksiz epitel hücreleri ise yaşlı, köşeli kenarlı ve çekirdeksiz olup "keratin çubuklar" oluşturabilirler. Vajinal lümenin distalindeki kalıcı keratinize yapılar veya çıplak elle dokunulması kontaminasyona yol açabilir. Ayrıca, idrar kontaminasyonu durumunda ürik asit kristalleri saptanırsa baskın hücre tipinin belirlenmesi etkilenebileceğinden yayma örneği değerlendirilmemelidir.
To determine the present cell types in vaginal cytology, prepared smear samples are examined under a light microscope, and Köhler illumination is utilized for optimum imaging. The examination begins with low magnification to differentiate cornified squamous epithelial cells, leukocytes, and nucleated epithelial cells, thereby determining the stage of the rat's estrus cycle. Neutrophils (leukocytes) are delicate cells with round, very small, and multi-lobed nuclei; they are distinguished from epithelial cells by their small size and irregular shapes. Nucleated epithelial cells possess a lightly stained cytoplasm and an oval nucleus. Small nucleated epithelial cells are round, non-keratinized, and have a high nucleus:cytoplasm ratio. On the other hand, large nucleated epithelial cells have abundant cytoplasm, a lower nucleus:cytoplasm ratio, a round to polygonal shape, and can be partially keratinized. Non-nucleated epithelial cells, also known as squames or "cornified" cells, are older, characterized by jagged or angular edges, lack a nucleus, and can form "keratin bars". Permanent keratinized structures in the distal vaginal lumen or touching the slide with bare hands can cause contamination. Additionally, in case of urine contamination, if uric acid crystals are detected, the smear sample should not be evaluated as it affects the determination of the predominant cell type.
