Dental amalgam, yüz yılı aşkın süredir düşük maliyeti, kolay uygulanabilirliği ve yüksek direnci sayesinde arka dişlerin restorasyonunda yaygın olarak kullanılan metalik bir materyaldir. Ancak içeriğindeki cıvanın potansiyel toksisitesi, tarih boyunca "amalgam savaşları" olarak adlandırılan büyük tartışmalara ve güvensizlik dönemlerine yol açmıştır. Yapılan kapsamlı bilimsel araştırmalar, çiğneme veya fırçalama gibi aktiviteler sırasında salınan cıva buharı miktarının uluslararası sağlık örgütleri tarafından belirlenen güvenli eşik değerlerin çok altında olduğunu kanıtlamaktadır. Epidemiyolojik ve klinik çalışmalar; dental amalgam varlığının yetişkinlerde böbrek fonksiyon bozukluğu, kronik yorgunluk, alzheimer veya multipl skleroz gibi nörodejeneratif hastalıklarla hiçbir nedensel bağı bulunmadığını ortaya koymuştur. Benzer şekilde, hamilelik sürecinde fetal gelişim ile doğum sonrasında çocukların nöropsikolojik ve renal işlevleri üzerinde olumsuz bir etkisi saptanmamıştır. Nadir görülen lokal alerjik reaksiyonlar ve hatalı atık yönetiminden kaynaklanan çevresel kaygılar dışında, mevcut bilimsel veriler cıva hijyenine uyulduğu sürece materyalin sistemik bir sağlık riski taşımadığını doğrulamaktadır. Sonuç olarak, fonksiyonel amalgam restorasyonların gereksiz yere sökülmesi diş yapısında geri dönüşsüz madde kaybına yol açtığından, materyalin klinik kullanımı geçerliliğini korumaktadır.

Dental amalgam has been widely used in posterior tooth restorations for over a century due to its low cost, ease of application, and high resistance. However, the potential toxicity of the mercury in its content has led to major controversies and periods of insecurity throughout history, termed as "amalgam wars". Extensive scientific research demonstrates that the amount of mercury vapor released during activities such as chewing or brushing is far below the safe threshold values determined by international health organizations. Epidemiological and clinical studies have revealed that the presence of dental amalgam has no causal link with neurodegenerative diseases such as multiple sclerosis or Alzheimer's, nor with chronic fatigue or renal dysfunction in adults. Similarly, no adverse effects have been detected on fetal development during pregnancy or on the neuropsychological and renal functions of children post-birth. Except for rare localized allergic reactions and environmental concerns arising from improper waste management, current scientific data confirms that the material poses no systemic health risk as long as mercury hygiene is strictly maintained. Consequently, since the unnecessary removal of functional amalgam restorations causes irreversible loss of tooth structure, the clinical use of this material remains entirely valid.
